Suggestion Form

You can submit your requests, suggestions, complaints and compliments about the Faculty through the notification form on the University's Integrated Quality Management System (BKYS). To make sure your submission reaches the Faculty, select TIP FAKÜLTESİ (Faculty of Medicine) in the Sorumlu Birim (responsible unit) field of the form.

The form opens on the University's Integrated Quality Management System (BKYS) and is in Turkish. The sender, notification type, responsible unit, subject, details, name and phone fields are required; e-mail and address are optional.

How to fill in the form

The form is in Turkish. The field names are given below together with their meaning.

  1. Bildirim Gönderen (sender): choose Personel (staff), Öğrenci (student) or Dış Paydaş (external stakeholder).

  2. Bildirim Türü (notification type): choose Öneri (suggestion), İstek (request), Şikayet (complaint), Memnuniyet (satisfaction), Bir Fikrim Var (I have an idea) or Diğer (other).

  3. Sorumlu Birim (responsible unit): select TIP FAKÜLTESİ (Faculty of Medicine). For hospital services, the list also contains a separate HASTANE (hospital) option.

  4. Konu and İçerik (subject and details): give a short subject and describe your request in the details field.

  5. Contact details: fill in Ad/Soyad (name) and Telefon (phone); E-posta (e-mail) and Adres (address) are optional. Tick "Cevap Verilmesini İstiyorum" if you would like to receive a reply.

  6. Submission: attach a related document under Doküman if needed, complete the "I'm not a robot" check and press Gönder (send).

As stated on the BKYS form, the feedback received is used to improve the management systems, and contact details are requested so that the improvements made can be shared with the person who submitted the feedback.
